New Patient Information
It is important to arrive 20 min early with a completed New Patient Packet of Information which you should have received prior to the appointment (or click the link to the right). Your initial visit will consist of a comprehensive physical examination and discussion of your health history. A personalized treatment plan will then be developed with you.
Note that we are a Comprehensive PM&R/Pain Management Center and offer a variety of services with a highly skilled team including Pain Management, Physical Therapy, MLS Laser Therapy, Holistic Medicine, Regenerative Medicine, and Medical Massage.
What do you need to bring to your first appointment?
- Picture I.D. (i.e. driver’s license, state I.D., etc.)
- Insurance Card(s)
- Insurance referral from primary care physician, if required
- Diagnostic Reports (i.e. x-ray, MRI, CT scan, bone scan, EMG studies, etc.)
- Completed New Patient Packet including a list of current medications
- Co-pay and deductible(s), if applicable
Before Your Visit
- If you have an HMO plan such as Blue Care Network, ask your primary care physician for a referral as soon as you book. We must receive it before your visit, or the appointment will need to be rescheduled.
- Have the facility that did your x-ray, MRI or CT scan fax the written report to (586) 563-3313. You do not need to bring films or discs.
- Fill out the New Patient Packet and list your current medications.
- We see patients age 12 and older.
What Happens at Your First Visit
- Arrive 20 minutes early to check in.
- Your doctor does a full physical exam and goes over your health history and any test results.
- Together you build a personalized treatment plan.
- Procedures are scheduled as a separate visit. If you were referred only for an EMG, it is done at the first visit.
- Your primary care physician and referring doctor receive a summary within one week, and your records are in the patient portal within 24 to 48 hours.
