New Patient Information
It is important to arrive 20 min early with a completed New Patient Packet of Information which you should have received prior to the appointment (or click the link to the right). Your initial visit will consist of a comprehensive physical examination and discussion of your health history. A personalized treatment plan will then be developed with you.
Note that we are a Comprehensive PM&R/Pain Management Center and offer a variety of services with a highly skilled team including Pain Management, Physical Therapy, MLS Laser Therapy, Holistic Medicine, Regenerative Medicine, and Medical Massage.
What do you need to bring to your first appointment?
- Picture I.D. (i.e. driver’s license, state I.D., etc.)
- Insurance Card(s)
- Insurance referral from primary care physician, if required
- Diagnostic Reports (i.e. x-ray, MRI, CT scan, bone scan, EMG studies, etc.)
- Completed New Patient Packet including a list of current medications
- Co-pay and deductible(s), if applicable
