How can I schedule an appointment?

Contact the office at (586) 563-3300  to request an appointment. 

The office is open Monday, Tuesday, Thursday, & Friday  9:00 a.m. - 5:00 p.m. Wednesday 9:00am - 6:00pm

 

 

What do you need to bring to your first appointment?

  • Picture I.D. (i.e. driver's license, state I.D., etc.)

  • Insurance card(s)

  • Insurance Referral from primary care physician, if required

  • Diagnostic Records (i.e. x-ray, MRI, CT scan, bone scan, EMG studies, etc.)

  • Completed New Patient Packet Including a List of current medications

  • Co-pay & deductible if applicable

What should you expect at your first appointment?

It is important to arrive 20 min early with a completed New Patient Packet of Information which you should have received prior to the appointment . Your initial visit will consist of a comprehensive physical examination and discussion of your health history.  A personalized treatment plan will then be developed  with you.  Note that we are a Comprehensive PM&R/Pain Management Center and offer a variety of services with a highly skilled team including Pain Management, Physical Therapy, MLS Laser Therapy, Holistic Medicine, Regenerative Medicine, and Medical Massage.

 

Procedures (i.e. acupuncture, injections, etc.) are generally not performed your first visit.

Dr. Wilson will continue to monitor your progress on a monthly basis, more frequent visits may be required if procedures are being performed. All findings will be forwarded to your primary care physician and the physician who referred you if it is a different physician.
